		Invoice is dated on the 1st of the month and will include all conference calls from the previous calendar month. Payment due net 30 days.

		There are NO sign up or monthly fees, pay only for the cost of usage at 7.9 cents per minute per participant. Optional Integrated web conferencing is FREE. Complete pricing details. Up to 10 Call Titles may be requested using this form. Additional Titles may be added to your account later. You may leave these spaces blank and we will activate a single Call Title using the Customer Name you provided above. If you request multiple Call Titles your invoice will be sub-totaled by Call Title.

Step 4: SELECT FREE, OPTIONAL SECURITY & OTHER FEATURES (Check all that apply, each feature can be deactivated at any time)


	FREE EMAIL INVITATION SERVICE -- Optional, use our free online conference invitation tool to send email invitations to participants for your conference calls.
	ROLL CALL -- capture participant names for additional security and accountability. Participants will be prompted to record their name when dialing into the conference call. Moderator can hear a roll call at any time by dialing *9.
	SECURITY PROMPT -- Allows the Moderator to enter a 4 to 9 digit security code when he/she starts a conference. All participants must then know this code to join the conference.
	BILLING CODES -- Allows the Moderator to enter a billing code before starting each call. Easily track spending and billing by assigning your own specific billing codes.
